Answer:  No. Even though processes operate in normal operating mode much of the time, many incidents occur during non-routine operations. 

2. Question: Does breakdown maintenance meet OSHA’s MI requirements? 

Answer: No. Preventive maintenance is required for critical equipment.

Answer:  C. 60 - 70%

Abnormal and non-routine operations were involved in 62% of incidents investigated by CSB through 2015.  In many cases, work was not recognized as a non-routine operation and was not reviewed to ensure that it could proceed safely. Flaws in hot work and other safe work practices also contributed to incidents, including lack of equipment isolation. 

4. Question: Should process safety information be kept in a central location? 

Answer: No. Process safety information needs to be kept near to where people need it. 

5. Question: Does new housing construction near a facility require an MOC review? 

Answer: This type of change is not addressed by US process safety regulations. However, offsite risks may have increased and it would be prudent to reassess them.
